During my 27 years of private practice, many patients have asked me if the ice against the heat should be used. I always start the conversation by explaining some basic facts. The main use of ice to reduce swelling or inflammation, not necessarily to reduce the pain. Ice can reduce the pain dull or numb the injured area, but it is mostly a pain reducer. Heat, on the other hand, has a calming effect on most musculoskeletal disorders.
Let's face it, patients come to my officefeel better. However, when heat is applied too soon, can actually cause more damage. That's why ... say that the sprained ankle. It is swollen? What would you use on a swollen ankle? You would have a heating pad on a swollen ankle? I doubt it. Most people would use the ice.
Now, if someone hurts your lower back, for example, shows the pain a little 'irritated. And if there are problems, there is a degree of swelling. Pt is typically receive more benefitsice in these circumstances, because the ice reduces swelling. This in turn reduces the amount of pressure on the nerves of the lower back.
I remember many years ago someone said the first 24 hours of ice and then heat. In my office I have another rule of thumb. Ice until there are signs of swelling or pain. In every doctor always consult your doctor determine the best treatment for your specific condition.
